We developed an intelligent hypertension care optimization toolkit by including several types of clinical and medical information, historical data on health conditions relating to patient and extended family, patient-level data arising from mobile health devices and wearable applications, as well as patient-reported data and data from social services. The solution acts as a layer for interactive real-time data collection and modelling of patients\u2019 overall health status. Hence, it aspires to determine effective personalized interventions and establish evidence-based therapies that promote patients\u2019 quality of life (including daily habits, psychosocial status, adherence to treatment plans, etc.). Finally, the scalable design of the system and the concrete interoperability layer will promote the capacity building of the solution and the trusted information exchange with pilot EHRs and PHRs. For national and regional health providers, proactive management of essential hypertension is key to limiting the growing resources required to tackle complications of hypertension. Hypertension is a major risk factor for coronary artery disease, stroke, heart failure, atrial fibrillation, and chronic kidney disease. At the same time, health and social care systems must develop a better integrated approach to enable more effective care delivery and to confront the complexity of different services which patients are now expected to navigate. It is widely recognized that health systems need to shift from treatment to health promotion and disease prevention, from a focus on disease to a focus on well-being and individuals, and from service fragmentation to the integration and coordination of services along the continuum of care. This shift can be supported by an ever-increasing volume of health data.
